This session will provide insight on current health care trends – which include a changing population, the prevalence of key chronic conditions and lifestyle risks, and the strains this puts on the health care system. Two physician leaders will share how they are shifting the industry to proactively address population health with an ability to zoom in to dynamically improve individual health and improve access to care via community-based care solutions.

Delve into the unique perspectives of these two physician leaders – one focused on population health solutions for payors and the other focused on community-based care delivery and access to care – as they discuss how these health trends create an urgency to deliver a sustainable impact in how we engage people to improve their health.
